Waiver and Order Form for DVD 2009 Boys State DVD   In Recent years Hoosier Boys State has produced a DVD that captures the thrill and vitality that is the Boys' State experience--allowing participants to remember their colleagues and experiences, and enabling families to participate in HBS vicariously.  Last year, for the first time, delegates actually participated in the shooting and editing of footage— adding perspective and verve to the film.  (This is no sleepy documentary for the faint of heart). The DVD always includes significant footage from the Talent Show.  Waivers/orders may be submitted any time during the week but presenting them at registration is encouraged.   1) Waiver Form (Everyone fill out please)   I give Hoosier Boys State permission to film and reproduce the image of (name)________________________.
